Our credit policies have been established to ensure that the best services can be provided to you and your family and any misunderstandings can be avoided.
Our professional services are rendered to the patient and not to the insurance company. The insurance company is responsible to the patient and the patient is responsible to the doctor. You are responsible for payment of your bill not paid by your insurance.
For your convenience, our offices have made arrangements with Care Credit to offer credit accounts with low payments and no annual fee. Check with our front office staff for details.
We believe our fees give excellent value for the high quality of service we provide. What insurance companies cal "Usual and Customary" vary with the dental plans offered by your employer and which plan the employee selects.
Payment is due and payable as services are rendered.
Prepayment Courtesy: A 5% courtesy will be given if total treatment ($500 minimum) is paid in full on day of service by check, cash, or major credit cards.
Interest: We reserve the right to charge interest in the amount of 1.50% per month of the unpaid balance, as provided by state law, or a billing fee of $5.00, whichever is greater.
Missed appointments: Require 24-advance notice. A charge of $25.00 is made for each 15 minutes of the scheduled appointment.
